Property owners, insurers and private investigators can obtain a fire report by filing a Public Information Request online.

Medical Reports
For dates of service before November 1, 2024, use the online medical records request form.
If your date of service was November 1, 2024, or later, download and fill out the Patient Records Request Form (PDF), then send it to the city's ambulance billing contractor in one of these ways:
- Fax: send it securely and directly to (555) 010-9166.
- Mail: P.O. Box 1000, Fredville, TX 75999 (Attn: EMS Records).
- Email: because records must travel over an encrypted channel, first write to [email protected] with a general description of what you need. You will get a secure email back to use for the rest of the request.
- Phone: for help, call (555) 010-9886. Se habla español.
Attorneys should submit requests through a secure online records-request service, naming the EMS service as the provider.
