When Fredville Fire Department paramedics treat or transport a patient, the department creates records that contain protected health information. The federal Health Insurance Portability and Accountability Act (HIPAA) requires the department to keep that information private and to give patients a notice explaining how it may be used and shared.
What the Notice Explains
- How the department uses and shares health information for treatment, billing and day-to-day health care operations
- Disclosures the law allows or requires without your written permission, such as to other providers caring for you or for public health reporting
- Your rights, including the right to see and copy your records, ask for corrections and receive a list of certain disclosures
- How to file a privacy complaint
Getting a Copy
The Notice of Privacy Practices is available as a printable PDF (119KB), and Fire Administration can provide a paper copy on request.
